Leave Paperwork

If your employer requires leave paperwork for the time you are away traveling and recovering, we are happy to complete that for you. Here are the steps to getting your leave paperwork taken care of.

1. No later than 90 before surgery, ask your employer to fax any required forms to us at 214-618-5506, or if they prefer, emailed to clinic1@hypospadias.com. Keep in mind some employers require you to fill out a personal section first, so they may send it to you first for that portion, and then have you send it along to us. This is still due 90 days before surgery.
2. No later than 90 before surgery, email Anusha Qureshi at clinic1@hypospadias.comand let her know you will need to have leave paperwork completed. She will send you our policy form for you to fill out and sign.
3. Pay your leave paperwork fee of $35.00 HERE. This is due no later than 90 days before surgery.

Please be sure that you provide the appropriate fax number or email where the completed forms should be sent. Once all required documents are received and the fee is paid, your paperwork will be completed and sent to your employer no later than 14 days before surgery.
